When you receive it, open the survey and follow the instructions shown on the screen. Answer all questions honestly and complete the survey to receive one entry into the sweepstakes.
Eligibility: Open only to legal residents of the United States (void in Puerto Rico, the U.S. Virgin Islands) who have reached the age of majority.
